Parents of children K-6th grade get ready for a week of Kids' Camp July 13-18th. Kids will spend a week at day camp playing, worshipping, studying God's Word, and taking day trips to various places around the county.  Drop off will begin at 8am. Camp ends at 4:30pm. If you require an earlier drop off or a later pick up time, please contact the church office at (336) 246-2012. CAMP IS FREE, IF YOU ALREADY PAID THERE WILL BE A LIST OF THOSE WHO PAID ON THE FIRST DAY ASKING YOU IF YOU WOULD LIKE A REFUND CHECK THE WEEK OF CAMP. 
Don’t worry, breakfast, lunch, snacks, and water are included.  There are days when towels, possible changes of clothes, water shoes, and sunscreen may be needed. It will work best to send these things all at once in a backpack or tote on Mon. or Tues. at the latest. You don’t want to miss this!  Register your child(ren) below. 
The full cost for camp is now FREE. This covers the cost of two meals, snacks, plenty of water and the extra afternoon field trips. Please contact Pastor Steve 336-246-2012 for scholarship information.
Deadline to register for camp is July 10th. We are approaching maximum CAPACITY AND WILL NOT BE ABLE TO ACCEPT REGISTRATIONS AFTER JULY 10TH. EVERY ITEM WITH AN ASTERISK NEEDS TO BE COMPLETED. YOU WILL RECEIVE AN EMAIL CONFIRMATION WHEN YOUR REGISTRATION GOES THROUGH WITHIN TWO BUSINESS DAYS. Please register only one child at a time.
 A list of all activities will be given to you when you bring your child/ren on the first day.

    Parents/Guardians, please note that you will receive a unique tag on the first day you drop off your child(ren) and be required to present it each day to pickup your child(ren). Your child will not be released unless the parent/guardian presents the tag.  Please acknowledge below.

    ​Parent/Guardian Consent and Indemnity Release Form.

    As the parent/legal guardian of the child listed above, I hereby consent for my child to attend and participate in all activities provided as described above for Midway Baptist Kid's Camp, 1670 Mt. Jefferson Rd. West Jefferson, N.C. 28694
    Note; Medication administration will only take place if the child(ren) is participating in an offsite event and must have the medication. Medication will only be administered as stated above by parent/guardian and/or a qualified professional.
    No exceptions will be made. 
    I authorize all medical and surgical treatment, X-ray, laboratory, anesthesia, and other medical and/or hospital procedures as may be performed or prescribed by the attending physician and/or paramedics and waive right to informed consent of treatment. I further agree to pay all charges for any medical care or treatment my child receives. This waiver applies only in the event that I, or the listed emergency contact, cannot be reached and my child is unable
    to make these decisions for him/herself.
    As a parent or legal guardian of my child, I am responsible for the health care decisions of my child and am authorized to consent to the services to be rendered. I represent that my consent to, and agreement to pay, for the medical care or treatment to be rendered to my child is legally sufficient and that no consent from any other person is required by law.
    I, the undersigned, fully recognize the dangers and hazards inherent in the activity listed above, and any related transportation, including personal injury, property damage, or wrongful death, as well as the unknown dangers and hazards which may arise in the course of my child’s participation, and do hereby voluntarily:
    Agree, for myself, my heirs, and my personal representative, to defend, hold harmless, indemnify, release, and forever discharge, to the broadest extent allowed by law, Midway Baptist Church, its trustees, officers, employees, servants, volunteers, agents, insurers, successors, assigns, from and against any and all claims, demands, action, or causes of action on account of any damage to real or personal property or any personal injury or death that may result from my child’s participation in the above mentioned activity.
    I have read this release, I understand it fully, I understand that it is legally binding, and I understand that, among other things, I am agreeing to indemnify Midway Baptist Church and its above-mentioned representatives, for injuries, damages or losses my child may cause and giving up rights to sue Midway Baptist Church and its above-mentioned
    ​representatives for injuries, damages, or losses incurred.

    PARENT/GUARDIAN SOCIAL MEDIA CONSENT/RELEASE FORM

    We are providing this parental/guardian social media consent & release form to both inform you and to request
    permission for your child's video/photo/image to be published on/in Midway's newsletter, bulletin, Facebook page,
    website or other social media outlets and publications.
    We will not be posting any personally identifiable information (e.g., names, address, etc.) However, the
    videos/photos/images are accessible since global access to the Internet does not allow us to control who may access such
    information. These dangers have always existed; however, we as a congregation do want to celebrate your child and
    his/her work. The law requires that we ask for your permission to use information about your child.
    Pursuant to law, we will not post any videos/photos/images without prior written consent from
    you as parent or guardian. Personally, identifiable information includes (in the case of Midway) video(s), photo(s) and
    image(s).
    If you, as the parent or guardian, wish to rescind this agreement, you may do so at any time in writing by
    sending a letter to the church administrator/staff person of Midway Baptist Church and such rescission will take effect
    upon receipt by the church administrator/staff person.​

    PARENT/GUARDIAN CONSENT TO MEDICAL, DENTAL, OR HOSPITAL CARE
    ​
    I consent to any x-ray examination, anesthetic, medical, or surgical diagnosis or treatment and
    hospital care under the general or special supervision and upon the advice of or to be rendered by a
    physician and surgeon licensed under the Medical Practice Act for my child. This authority also
    extends to any x-ray examination, anesthetic, dental, or surgical diagnosis or treatment and hospital care
    by a dentist licensed under the Dental Practice Act for my child. I further agree to pay all charges for
    the dental, medical, or hospital care or treatment.
    As parent or legal guardian of my child, I am responsible for the health care decisions of my
    child and am authorized to consent to the services to be rendered. I represent that my consent to and
    agreement to pay for the dental, medical, or hospital care or treatment to be rendered to my child is
    legally sufficient and that no consent from any other person is required by law.
